  • Banking Information: Bank name, bank address, account holder name, account type, routing number, and account number
  • Documents: Voided check, driver's license (front and back), W-9 form, insurance certificate, MC authority, ACH verification, and NOA release letter (if applicable)
  • Signature: Electronic signature and date
  • Communication Preferences: SMS consent status and phone number for text notifications
